目的　调查天津市平津医院妇科2008～2010年不孕症住院患者情况，为进一步研究提供基线数据。
方法　按WHO不孕症诊断标准，收集天津市平津医院妇科2008～2010年不孕症住院患者的的人口学特征、病因和费用构成，而后采用ACCESS数据库录入数据和SPSS 13.0软件进行统计分析。
结果　① 该院妇科2008～2010年不孕症住院患者共1 452例，来自全国33个地区，79.7%来自北方。② 患者平均年龄31.2±4.3岁，其中，30～34岁年龄段患者最多，占40.3%；脑力劳动者较体力劳动者多（23.3% vs. 7.2%）；有大学文化水平的患者最多，占36.7%；家庭年收入在2～19万之间的患者最多（83.1%）。③ 初次性生活的平均年龄21.4±2.9岁，53.7%的患者只有1位性伴侣，夫妻每周性生活次数2次者最多。丈夫年龄平均32.9±5.5岁，30～35岁（39.2%）的比例最大，丈夫职业为脑力劳动者最多（35.9%）；64.9%精液分析正常，23.0%未查。④ 不孕症病因中原发不孕占29.9%，继发不孕占70.1%，其中57.6%的患者曾行人流或药流。患者不孕时间1～21年，平均5.2±3.5年。⑤ 76.3%的患者合并有盆腔粘连，88.6%的患者输卵管有问题，合并子宫问题占23.6%，合并卵巢问题占5.2%，合并子宫内膜异位症占5.0%，合并多种问题占6.7%。不明原因占4.8%。1 286例输卵管性不孕的患者中，输卵管远端闭锁的发生率（45.8%）多于近端阻塞（32.9%）；远端闭锁合并输卵管积水的占24.7%，单纯闭锁无积水的占21.1%，输卵管先天发育异常占15.2%。⑥ 患者平均住院日10.5天，人均住院费用14 253.3元。其中材料费占29.1%，药品费占18.2%。
结论　① 2008～2010年天津市平津医院妇科共收治不孕症住院患者1 452例。其中79.7%来自北方，年龄以30～34岁居多，44.3%无工作，大学文化水平所占比例最高，绝大多数为中低收入家庭。性生活较保守，丈夫年龄30～35岁居多，脑力劳动者多，23.0%未行精液检查。继发不孕较多，不孕时间1～21年。② 76.3%的患者合并有盆腔粘连，输卵管性不孕占88.6%，且远端闭锁多于近端阻塞。③ 平均住院日10.5天，人均住院费用14 253.3元，远低于辅助生殖技术每个治疗周期的费用。费用构成以材料费和药品费为主（47.3%），以自费为主。
