目的 探討腹腔鏡下胃轉(zhuǎn)流術(shù)治療肥胖型2型糖尿病的短期和長期臨床效果。
方法 回顧性分析四川大學(xué)華西醫(yī)院2009年11月至2012年2月期間開展腹腔鏡下胃轉(zhuǎn)流手術(shù)治療肥胖型2型糖尿病20例患者的病例資料,根據(jù)隨訪時(shí)間的不同,分為<1年(20例)的短期療效分析和≥1年(11例)的長期療效分析兩個(gè)部分。觀察術(shù)后患者體重、血糖、糖化血紅蛋白、胰島素抵抗水平、血壓和血脂控制情況。計(jì)算糖尿病短期有效率和長期緩解率,并分析影響糖尿病緩解的因素。
結(jié)果 術(shù)后1年內(nèi),患者平均體重、血糖和糖化血紅蛋白均明顯降低,胰島素抵抗減輕,手術(shù)對高血壓和血脂紊亂也有明顯治療效果,糖尿病短期有效率為90.0%(18/20),短期內(nèi)血糖和用藥均達(dá)完全緩解標(biāo)準(zhǔn)的患者比例為9/20(45.0%),糖尿病病程和術(shù)前C-肽水平與短期緩解率有關(guān)。長期隨訪病例,術(shù)后平均體重和血糖、糖化血紅蛋白明顯降低,胰島素抵抗減輕,高血壓和血脂紊亂得到有效控制,手術(shù)使糖尿病長期緩解者的比例為9/11(81.8%),臨床治愈者比例為6/11(54.5%),糖尿病病程和空腹血糖水平與糖尿病緩解有關(guān)。手術(shù)不良反應(yīng)少,患者滿意度較高。
結(jié)論 腹腔鏡下胃轉(zhuǎn)流術(shù)治療肥胖型2型糖尿病的短期和長期臨床效果明顯,是較為安全而有效的外科干預(yù)糖尿病的方式。
