目的 　探討Ⅳ型肝門部膽管癌的初步治療經(jīng)驗。
方法 　收集2008年4月至2011年4月期間四川大學華西醫(yī)院肝膽胰外科1個醫(yī)療組收治的20例Ⅳ型肝門部膽管癌患者的臨床資料，對其術中及術后結果進行分析。
結果 　20例中行手術切除15例，切除率為75%。其中行根治性切除術10例，非根治性切除術5例；行左三肝＋尾葉切除（右前右后肝管成形、肝管-空腸吻合＋空腸-空腸Roux-en-Y吻合術）7例，行擴大右半肝切除＋尾葉切除（左外左內(nèi)肝管成形、肝管-空腸吻合＋空腸-空腸Roux-en-Y吻合術）6例，行肝方葉切除（左外內(nèi)、右前后肝管分別整形后行2個膽-腸吻合術）2例；其中2例因總膽紅素＞400 mmol/L而先行經(jīng)皮肝穿刺膽管引流（PTCD）和門靜脈栓塞（PVE）后再手術，1例行根治性切除術，另1例行非根治性切除術。不能切除者5例，其中3例因發(fā)生肝臟及遠處廣泛轉(zhuǎn)移，行術中PTCD，另2例因腫瘤侵犯門靜脈左右支，行術中T管引流、聯(lián)合術后PTCD。行手術切除者術后1年生存率為100%，術后生存時間1～3年，平均1.5年；腫瘤未切除者生存3～6個月，平均4.2個月。本組無一例圍手術期死亡。
結論 　對于Ⅳ型肝門部膽管癌，術前應精確評估，力爭切除腫瘤，解除膽管梗阻；如不能一期切除則行PTCD和PVE，力爭二期切除。
