目的探討肝移植大鼠術(shù)后早期肝竇內(nèi)皮細(xì)胞功能的改變及前列腺素E1(PGE1)對其的保護(hù)作用。方法用“兩袖套法”建立大 鼠原位肝移植模型,其中A組為正常大鼠空白對照組,B組為正常大鼠供體肝移植手術(shù)對照組,C組為休克性大鼠供體肝移植實(shí)驗(yàn)對照 組,D組為PGE1保護(hù)的休克性大鼠供體肝移植實(shí)驗(yàn)組,每組各8只。肝移植各組(無特別說明均指受體)于術(shù)后6 h取血,檢測其血清 中的肝臟酶學(xué)(ALT、LDH)、丙二醛(MDA)和一氧化氮(NO),以及血漿內(nèi)皮素(ET)水平,并取肝組織作常規(guī)病理學(xué)檢查,比較 各組有無差異。結(jié)果各移植組供肝冷保存時(shí)間及無肝期均接近,分別為(2±0.5) h和(15±3) min; B、D組術(shù)后6 h全部存活, 存活率100%; C組術(shù)后6 h存活5只,存活率62.5%。與C組比較,D組ALT、LDH、MDA、ET明顯降低(P<0.05),NO明顯升高(P<0.05 )。C組肝臟病理學(xué)檢查發(fā)現(xiàn)有明顯的組織學(xué)損害,而B、D組肝臟組織結(jié)構(gòu)基本完好。結(jié)論肝移植可造成肝竇內(nèi)皮細(xì)胞明顯損害,血 清NO和血漿ET水平可較好地反映肝竇內(nèi)皮細(xì)胞的功能狀態(tài)。PGE1通過穩(wěn)定血流動力學(xué)和穩(wěn)定肝竇內(nèi)皮細(xì)胞質(zhì)膜,可減輕移植肝肝竇內(nèi) 皮細(xì)胞的損害。
