目的 總結(jié)法洛四聯(lián)癥（TOF）合并冠狀動脈畸形（ACA）手術(shù)治療經(jīng)驗，探討ACA的診斷，右心室流出道（RVOT）疏通手術(shù)方式的選擇及其效果?！》椒ā』仡櫺苑治?004年1月至2010年1月武漢亞洲心臟病醫(yī)院手術(shù)矯治TOF合并ACA 29例，其中男18例，女11例；年齡7歲（5個月～33歲），體重18 （5～51） kg。術(shù)前動脈血氧飽和度65%～91%。所有患者均行完全矯治手術(shù)。1例經(jīng)右心房疏通RVOT，修補(bǔ)室間隔缺損；3例經(jīng)右心房疏通RVOT，修補(bǔ)室間隔缺損，用自體心包片加寬主肺動脈；3例行肺動脈移位術(shù)（REV），5例患者采取“雙通道”手術(shù)； 11例采取冠狀動脈下方、上方或者兩邊切口疏通，補(bǔ)片加寬的方法（“單片”或“雙片”法）；6例行冠狀動脈移植術(shù)后行跨瓣環(huán)補(bǔ)片重建RVOT。 結(jié)果 體外循環(huán)時間78 （65～102） min，主動脈阻斷時間50 （40～82） min，手術(shù)時間150 （126～178） min。手術(shù)未出現(xiàn)死亡以及嚴(yán)重冠狀動脈損傷。術(shù)后2例因出血行二次開胸止血；2例出現(xiàn)低心排血量，強(qiáng)心藥物支持后治愈。隨訪51 （21～83）個月，隨訪過程中無死亡。所有患者術(shù)后心功能（NYHA）分級均為Ⅰ級，心臟射血分?jǐn)?shù)均正常，心電圖未提示心肌缺血，動脈血氧飽和度恢復(fù)至96%～99%。術(shù)后早期RVOT壓差（△P）為19 （8～38） mm Hg，術(shù)后隨訪患者無△P增加。 結(jié)論 對于TOF合并ACA患者，術(shù)前通過64排螺旋CT （64-MSCT）檢查可以明確ACA診斷，術(shù)中根據(jù)冠狀動脈情況選擇合適的流出道疏通方法，手術(shù)效果良好。
