目的 總結(jié)血液或腹膜透析患者行心血管手術(shù)的臨床經(jīng)驗(yàn),評(píng)價(jià)其安全性和有效性?!》椒ā』仡櫺苑治?004年12月至2011年4月北京協(xié)和醫(yī)院連續(xù)10例維持性血液或腹膜透析患者行心血管手術(shù)治療的臨床資料,其中男6例,女4例;年齡23~71 (57.6±13.2)歲。腎功能衰竭的原因?yàn)樘悄虿∧I病6例,慢性腎小球腎炎3例,系統(tǒng)性紅斑狼瘡腎病1例。術(shù)前透析時(shí)間3~98 (25.2±30.6)個(gè)月;血液透析8例,腹膜透析2例。行冠狀動(dòng)脈旁路移植術(shù)5例,Bentall手術(shù)1例,主動(dòng)脈瓣置換術(shù)2例,二尖瓣置換術(shù)1例,上腔靜脈取栓+補(bǔ)片加寬成形術(shù)1例。擇期手術(shù)前1 d均透析1次,術(shù)中體外循環(huán)期間持續(xù)超濾。距手術(shù)5~32 h后開(kāi)始行床旁持續(xù)無(wú)肝素血液濾過(guò)透析,4~7 d后轉(zhuǎn)為常規(guī)血液或腹膜透析?!〗Y(jié)果 所有患者手術(shù)均順利完成,體外循環(huán)時(shí)間(125.8±33.5) min,主動(dòng)脈阻斷時(shí)間(77.2±25.5) min。住院期間因胸部切口感染致感染性休克死亡1例。1例術(shù)后發(fā)生心臟壓塞,緊急二次開(kāi)胸止血;3例發(fā)生心房顫動(dòng),給予胺碘酮治療后轉(zhuǎn)復(fù)為竇性心律。隨訪(fǎng)9例,隨訪(fǎng)時(shí)間8~76個(gè)月,隨訪(fǎng)期間因顱內(nèi)出血和肝癌死亡2例;7例生存患者心功能均為Ⅱ級(jí),未發(fā)生心絞痛、心肌梗死或人工瓣膜異常等心血管相關(guān)不良事件。1例腹膜透析患者于出院后14個(gè)月因腹膜炎轉(zhuǎn)為血液透析?!〗Y(jié)論 維持性血液或腹膜透析患者經(jīng)過(guò)正確的圍術(shù)期處理,可以接受心臟大血管手術(shù),從而改善臨床癥狀、提高生活質(zhì)量。
