目的 了解心臟手術(shù)后急性腎損傷(AKI)的發(fā)病及預(yù)后情況,探討急性腎損傷網(wǎng)絡(luò)(AKIN)會(huì)議推薦的AKI分期預(yù)測患者院內(nèi)死亡的應(yīng)用價(jià)值?!》椒ā?004年1月至2007年6月上海交通大學(xué)醫(yī)學(xué)院附屬仁濟(jì)醫(yī)院收治的所有成年心臟手術(shù)患者1 056例納入研究,采用AKIN推薦的AKI定義及分期標(biāo)準(zhǔn)評(píng)估心臟手術(shù)后AKI的發(fā)病率及住院病死率,并采用單因素和logistic多因素回歸分析法對(duì)術(shù)前、術(shù)中、術(shù)后與AKI發(fā)生可能相關(guān)的危險(xiǎn)因素進(jìn)行分析?!〗Y(jié)果 在1 056例行心臟手術(shù)的患者中,328例發(fā)生AKI,發(fā)生率為31.06%;AKI患者的住院病死率顯著高于非AKI患者(11.59% vs. 0.69%,P<0.05)。Logistic多因素回歸分析顯示:年齡每增加10歲(OR=1.40)、術(shù)前高尿酸血癥(OR=1.97)、術(shù)前左心功能不全(OR=2.53)、冠狀動(dòng)脈旁路移植術(shù)(CABG)加心瓣膜手術(shù)(OR=2.79)、手術(shù)時(shí)間每增加1 h (OR=1.43)和術(shù)后循環(huán)血容量不足(OR=11.08)是心臟手術(shù)后發(fā)生AKI的獨(dú)立危險(xiǎn)因素。AKIN分期預(yù)測患者院內(nèi)死亡的ROC曲線下面積為0.865,95%可信區(qū)間為0.801-0.929。 結(jié)論 隨著AKIN分期的上升,心臟手術(shù)患者住院病死率逐步升高。年齡高、術(shù)前高尿酸血癥、術(shù)前左心功能不全、CABG加心瓣膜手術(shù)、手術(shù)時(shí)間延長和術(shù)后循環(huán)血容量不足是心臟手術(shù)后并發(fā)AKI的獨(dú)立危險(xiǎn)因素。AKIN分期可以有效預(yù)測心臟手術(shù)患者發(fā)生院內(nèi)死亡的風(fēng)險(xiǎn),為及早對(duì)高危人群采取有效的預(yù)防干預(yù)措施提供依據(jù)。
