目的 總結(jié)采用血管旁路移植術(shù)聯(lián)合腔內(nèi)隔絕術(shù)(endovascular aortic repair,EVAR)治療主動(dòng)脈擴(kuò)張疾病的臨床經(jīng)驗(yàn)。 方法 2008 年1 月- 2011 年8 月,采用血管旁路移植術(shù)聯(lián)合EVAR 治療12 例主動(dòng)脈擴(kuò)張疾病患者。其中男11 例,女1 例;年齡47 ~ 81 歲,平均65.9 歲。均經(jīng)主動(dòng)脈血管造影(computed tomography angiography,CTA)確診,其中Stanford A 型夾層1 例,Stanford B 型夾層5 例,主動(dòng)脈弓部動(dòng)脈瘤4 例,腹主動(dòng)脈瘤2 例。8 例患者先行頸部血管路移植術(shù),再行EVAR;4 例患者先行EVAR,再行股動(dòng)脈血管旁路移植術(shù)。 結(jié)果 術(shù)后肺部感染3 例,腎功能不全2 例,腦梗死1 例,血紅蛋白和血小板降低7 例,腹股溝切口愈合不良1 例。11 例患者獲隨訪3 ~ 42 個(gè)月,平均18.6 個(gè)月。1例行胸、腹主動(dòng)脈EVAR 患者,術(shù)后6 個(gè)月復(fù)查發(fā)現(xiàn)移植物遠(yuǎn)端有新的動(dòng)脈瘤形成,再次行EVAR,3 個(gè)月后復(fù)查恢復(fù)良好。其余患者術(shù)后3、6 及12 個(gè)月復(fù)查,主動(dòng)脈CTA 均提示假腔明顯縮小以及假腔內(nèi)血栓形成,未發(fā)現(xiàn)內(nèi)漏,支架無遲發(fā)變形或移位,人工血管吻合口通暢。 結(jié)論 血管旁路移植術(shù)聯(lián)合EVAR擴(kuò)大了腔內(nèi)修復(fù)術(shù)的適應(yīng)證,既提供了足夠的錨定區(qū),又保證了重要臟器血供,簡化了手術(shù)操作過程,降低了腔內(nèi)治療難度。
