目的 　評(píng)價(jià)生長(zhǎng)抑素及其類似物能否降低內(nèi)鏡逆行胰膽管造影術(shù)(ERCP)后胰腺炎(PEP)的發(fā)生率及嚴(yán)重程度,以及臨床應(yīng)用的安全性.
方法 　計(jì)算機(jī)檢索、Cochrane臨床對(duì)照試驗(yàn)試驗(yàn)資料庫(kù)(2004年第1期)、MEDLINE(1966～2004.4)、EMBASE(1985～2004.4)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)(1970～2004.4)和中國(guó)循證醫(yī)學(xué)中心隨機(jī)對(duì)照試驗(yàn)資料庫(kù),并輔以手工檢索,全面收集全世界關(guān)于生長(zhǎng)抑素及其類似物預(yù)防PEP的RCT,按Cochrane協(xié)作網(wǎng)推薦的方法進(jìn)行系統(tǒng)評(píng)價(jià).
結(jié)果 　共31篇RCT包括4 728例患者被納入評(píng)價(jià).Meta分析結(jié)果顯示,生長(zhǎng)抑素可降低PEP的發(fā)生率[OR 0.33,95%CI (0.20,0.54);P=0.000 01;NNT＝13];與安慰劑或空白對(duì)照試驗(yàn)相比,生長(zhǎng)抑素類似物奧曲肽可降低ERCP術(shù)后高淀粉酶血癥的發(fā)生率[OR=0.54,95%CI (0.38,0.77);P=0.000 7],但對(duì)PEP、重度PEP和ERCP術(shù)后腹痛的發(fā)生率無影響.
結(jié)論 　生長(zhǎng)抑素可降低PEP的發(fā)生率,可能是預(yù)防PEP的有效方法.由于納入的研究數(shù)量較多,有4篇為高質(zhì)量的研究,且敏感性分析的結(jié)果與Meta分析的結(jié)果一致,上述結(jié)論具有一定的可信度.但目前的證據(jù)不支持奧曲肽降低PEP的發(fā)生率,故不推薦臨床用于PEP的預(yù)防.鑒于敏感性分析的結(jié)果提示奧曲肽甚至有增高PEP發(fā)生率的趨勢(shì),是否有必要進(jìn)行進(jìn)一步的臨床試驗(yàn)需審慎看待.
