目的 　探討可能導(dǎo)致直腸癌術(shù)后局部復(fù)發(fā)的臨床病理因素。
方法 　應(yīng)用單因素和多因素分析方法，回顧性分析2004年1月至2011年4月期間我院收治的368例直腸癌患者完整臨床及病理資料。
結(jié)果 　局部復(fù)發(fā)73例，占同期直腸癌（368例）的19.84%。單因素分析結(jié)果顯示，直腸癌患者的性別、腫瘤距肛緣距離、腫瘤周徑、TNM分期、組織學(xué)類型、脈管浸潤、腫瘤穿孔、吻合口漏及放化療與術(shù)后局部復(fù)發(fā)有關(guān)（P＜0.05），而與年齡、病程、大體類型、腫瘤直徑及手術(shù)方式無關(guān)（P＞0.05）。進(jìn)一步多因素分析結(jié)果顯示，腫瘤距肛緣距離、腫瘤周徑、TNM分期、組織學(xué)類型、脈管浸潤、腫瘤穿孔及放化療是術(shù)后局部復(fù)發(fā)轉(zhuǎn)移的危險(xiǎn)因素（P＜0.05）。
結(jié)論 　直腸癌術(shù)后局部復(fù)發(fā)與多種因素有關(guān)，脈管浸潤是最重要的危險(xiǎn)因素。
