目的  總結(jié)達芬奇機器人手術(shù)系統(tǒng)在高齡重度梗阻性黃疸患者中的治療經(jīng)驗。
方法  第二炮兵總醫(yī)院肝膽胃腸病研究所在2009年1月至2011年5月期間共行機器人手術(shù)209例,其中高齡重度梗阻性黃疸患者47例(機器人組),同期開腹手術(shù)患者44例(開腹組),對比觀察患者疾病分布,術(shù)前、術(shù)中及術(shù)后等相關(guān)指標,對其結(jié)果進行歸納分析。
結(jié)果  2組患者基線一致(P>0.05)。機器人組46例在機器人輔助下完成手術(shù),手術(shù)完成率達97.87%(46/47),1例中轉(zhuǎn)開腹,中轉(zhuǎn)率為2.13%(1/47);手術(shù)方式包含肝膽胰外科所有手術(shù)方式,且其中1例肝門部膽管癌患者因血清膽紅素水平極高(375μmol/L),急診行肝門部膽管癌切除+肝門-空腸Roux-en-Y吻合術(shù);術(shù)后有7例患者出現(xiàn)并發(fā)癥,總體并發(fā)癥發(fā)生率為14.89%(7/47);圍手術(shù)期死亡1例,死亡率為2.13%(1/47);術(shù)后3個月生存率為78.15%;術(shù)后15 d和術(shù)后3個月體力狀況評分分別為42.87±18.61和58.51±23.86;腹部切口總長度為(6.30±1.70) cm。開腹組所有患者均完成手術(shù),其中急診手術(shù)4例;術(shù)后13例患者出現(xiàn)并發(fā)癥,總體并發(fā)癥發(fā)生率為29.55%(13/44),圍手術(shù)期死亡2例,死亡率為4.55%(2/44);術(shù)后3個月生存率為72.36%;術(shù)后15d和術(shù)后3個月體力狀況評分分別為37.15±13.64和45.27±18.96;腹部切口總長度為(26.73±3.07) cm。2組總體并發(fā)癥發(fā)生率、術(shù)后15d和術(shù)后3個月體力狀況評分及切口總長度比較,差異均有統(tǒng)計學(xué)意義(P<0.05)。
結(jié)論  達芬奇機器人手術(shù)系統(tǒng)對于高齡重度梗阻性黃疸患者的治療是安全、有效的,其創(chuàng)傷小,操作靈活精準,術(shù)后恢復(fù)快,拓展了外科手術(shù)的年齡界限,擴大了手術(shù)適應(yīng)人群。
