目的應(yīng)用激光多普勒血流探測儀(LDF)和激光多普勒血流成像儀(LDPI)監(jiān)測Lewis大鼠后肢急性缺血模型血流和血壓的動(dòng)態(tài)變化,探討大鼠后肢急性缺血后血流變化特點(diǎn)。方法切除大鼠左后肢股動(dòng)脈制備急性后肢缺血模型,于術(shù)后2、7、14、28及49 d對手術(shù)側(cè)和非手術(shù)側(cè)肢體采用LDF進(jìn)行血流、血壓檢測,于術(shù)后7 d采用LDPI進(jìn)行血流檢測。結(jié)果所有大鼠術(shù)后均成活,未發(fā)現(xiàn)后肢壞死; 在術(shù)后14 d內(nèi)手術(shù)側(cè)后肢平均分為2分,在49 d平均分為1分。大鼠手術(shù)側(cè)和非手術(shù)側(cè)肢體血流比值在術(shù)后2 d由術(shù)前的1上升至1.31±0.439(P=0.021),術(shù)后7 d和14 d分別為0.82±0.538和0.93±0.294,兩者比較差異無統(tǒng)計(jì)學(xué)意義(P=0.502),但均明顯低于術(shù)后2 d的值(P=0.032和P=0.019); 術(shù)后28 d下降到最低點(diǎn)(0.41±1.970),明顯低于術(shù)后2、7和14 d值(P=0.004、P=0.007和P=0.006); 在術(shù)后49 d手術(shù)側(cè)后肢血流恢復(fù)到接近術(shù)前值(0.98±0.093),明顯低于術(shù)后2 d(P=0.010)而高于術(shù)后28 d值(P=0.005),與術(shù)后7 d和14 d的差異無統(tǒng)計(jì)學(xué)意義(P=0.126和P=0.382)。大鼠手術(shù)側(cè)和非手術(shù)側(cè)肢體血壓比值術(shù)后2 d由術(shù)前的1明顯下降至0.47±0.375(P=0.031); 術(shù)后7 d繼續(xù)下降至0.44±0.118,與術(shù)后2 d比較差異無統(tǒng)計(jì)學(xué)意義(P=0.203); 術(shù)后14 d下降到最低點(diǎn)(0.35±0.115),明顯低于術(shù)后2 d和7 d值(P=0.001和P=0.036); 術(shù)后28 d開始上升(0.54±0.146),明顯高于術(shù)后14 d值(P=0.008),但與術(shù)后2 d(P=0.493)和7 d(P=0.551)的差異無統(tǒng)計(jì)學(xué)意義; 術(shù)后49 d恢復(fù)接近術(shù)前值(0.97±0.094),明顯高于術(shù)后2、7、14和28 d值(P=0.013、P=0.021、P=0.002和P=0.031)。結(jié)論切除大鼠后肢股動(dòng)脈及分支的方法制備后肢急性缺血模型,在術(shù)后14~28 d患肢缺血處于最嚴(yán)重階段。LDF和LDPI可以動(dòng)態(tài)監(jiān)測肢體血流及血壓的變化,對監(jiān)測大鼠后肢術(shù)后缺血的動(dòng)態(tài)演變過程有著重要的作用。
