目的 探討D-二聚體和彩色多普勒超聲在下肢深靜脈血栓形成（DVT）中的診斷價值。
方法 回顧性分析我科2008年1月至2010年4月期間收治的70例臨床診斷為下肢DVT患者的臨床資料，記錄血漿D-二聚體檢測和彩色多普勒超聲檢查結(jié)果，同時比較深靜脈血栓形成不同時期和不同分型中D-二聚體水平變化。
結(jié)果 D-二聚體檢測和彩色多普勒超聲檢查診斷下肢靜脈血栓的敏感度、特異度、陽性預(yù)測值、陰性預(yù)測值和準(zhǔn)確度分別為100%、66.7%、97.0%、100%、97.1%和98.4%、83.3%、98.4%、83.3%、97.1%。血漿D-二聚體水平在深靜脈血栓形成的急性期〔(6 451±4 012.22) μg/L〕和亞急性期〔(2 063±1 831.35) μg/L〕明顯高于正常對照組水平〔(310±66.70) μg/L〕，P lt;0.01和P lt;0.05，而慢性期患者的D-二聚體水平〔(466±350.52) μg/L〕與正常對照組相比差異無統(tǒng)計學(xué)意義（P gt;0.05）。中央型〔(2 149±1 911.53) μg/L〕和混合型〔(4 464±3 753.16) μg/L〕患者血漿D-二聚體水平明顯高于正常對照組（P lt;0.05和P lt;0.01），周圍型患者血漿D-二聚體水平〔(560±315.62) μg/L〕接近正常對照組水平（P gt;0.05）。
結(jié)論 彩色多普勒超聲檢查的敏感性及特異性較高，可作為下肢DVT患者的首選檢查； D-二聚體在急性DVT患者中敏感性較高，可作為DVT的預(yù)測指標(biāo)。
