【摘要】目的 探討肝移植門靜脈再灌注過程中肝動脈缺血(hepatic artery ischemia,HAI)損傷的嚴重性和應用肝動脈橋式置管(hepatic artery bridgeconduit,HABC)技術實現(xiàn)門靜脈、肝動脈同時灌注對這一損傷的保護作用。
方法 32只犬按隨機數(shù)字表法隨機均分為4組: 正常對照組、HAI 30 min組、HAI 2 h組和HABC組。后三組分別建立犬自體原位肝臟移植模型,HABC組應用HABC技術使肝動脈、門靜脈同時再灌注。術后取供肝組織與膽管組織電鏡觀察肝細胞和膽管上皮細胞病理學改變。分別應用硫代巴比妥酸法檢測肝組織中丙二醛(MDA)濃度、黃嘌呤氧化酶法檢測過氧化物歧化酶(SOD)活性、酶標記法檢測肝細胞線粒體琥珀酸脫氫酶(SDH)活性。
結果 HAI 30 min即可見供肝肝細胞和膽管上皮細胞水腫、線粒體嵴減少,HAI 2 h其病理改變進一步加重,膽管上皮尤為明顯,而HABC組則見肝細胞和膽管上皮細胞比較完整,膽管上皮絨毛豐富。HAI 30 min組和HAI 2 h組肝組織中MDA含量增加,分別為(1.652±0.222) nmol/mg prot和(2.379±0.526) nmol/mg prot,而SOD則降低至(11.15±3.9) U/mg prot和(9.47±3.4) U/mg prot,SDH活性則分別降低至0.362±0.019和0.281±0.029,與正常對照組比較差異有統(tǒng)計學意義(P<0.05,P<0.01); 而HABC組MDA含量、SOD和SDH活性與正常對照組比較差異無統(tǒng)計學意義(P gt;0.05)。
結論 HABC技術的實施,可為臨床肝移植中預防HAI損傷,減少肝移植術后并發(fā)癥,特別是膽道并發(fā)癥的發(fā)生提供有效的方法。
